I want someone to build a intake form in excel - it will collect Name, phone, addresses etc. It will require a couple of drop down cells and as each field of data being requested on the intake form it will go to the next field when you hit enter or tab. On two areas it requires a pop up Calendar to pick a date that is being requested. I need the entered data to be collected in the back ground and another sheet in a format that can be connected to another system like ClickUp.
