I am looking to create a multi-step form for an insurance company. The requirements for the form will be explained below. The design of the form will be completed by me so this will only be development work. My plan is to use WordPress, this would be ideal because WordPress is a platform I commonly use however if you feel another platform would be better suited for this project I am open to your recommendations.
Requirements
The form will be split into 3 parts;
1. Insurance options 2. Enter personal details (name, address, etc) 3. Generate digital contract
At the end of the form I want to send a SMS message to the visitor with a link they can open on their mobile to draw their signature and upload a picture of their ID. Once this information is submitted the visitor can go back to the form page and confirm they have submitted their details.
The page should update (ideally with AJAX so the page does not refresh) then an option will appear for the client to open a PDF of their digital insurance contract. This will basically include all the options they selected in the first insurance step, their personal details as well as their signature and ID added in the relevant places on the PDF.
I am looking for the development budget as soon as possible ideally by tomorrow, Saturday or latest Sunday please.
